As a case manager, Heather works with patients who鈥檝e been in and out of hospital at least three times in a year. Her work is unusual for a pharmacist: typically, case managers are nurses or social workers, but Heather鈥檚 prior experience as a hospital pharmacist helps her collaborate with patients and their healthcare team to identify why hospitalizations occur and plan to prevent them.

Greg Becotte is a pharmacist at the Pier Health Resource Center in Vancouver鈥檚 Downtown Eastside (DTES). The DTES is a neighbourhood notorious for high rates of poverty, mental illness, crime, and infectious disease.

Every Friday, you鈥檒l find Khrystine (Rx2018) at Wilfrid Laurier Sports Complex helping athletes with intellectual disabilities improve their swimming skills. She鈥檚 an assistant swim coach with Special Olympics Ontario, a non-profit, charitable organization that provides sports training and competitions for those with intellectual disabilities.
