Verification of Illness (VIF) Services

University of À¶Ý®ÊÓƵ verification of illness certificates are available for students who are unable to fulfill academic obligations.Ìý

What you need to know

Campus Wellness Process

  1. Attend University of À¶Ý®ÊÓƵ Campus Wellness during the illness to receive appropriate medical care, an accurate diagnosis and to receive a Verification of Illness Form (VIF) that will be completed by your healthcare practitioner.  
  2. Campus Wellness will complete and submit the VIF for you to the Absence Database.Ìý
  3. Please be aware that your Campus Wellness healthcare practitioner may charge a $20.00 fee for completion of the Verification of Illness.Ìý 
  4. Your instructors will be automatically notified of your submitted absence. Contact them promptly to discuss accommodations for missed work, as the final decision is theirs.Ìý

Off-campus Practitioner Process  

  1. Attend your family healthcare practitioner during the illness to receive appropriate medical care, and an accurate diagnosis.Ìý

  2. Bring the University of À¶Ý®ÊÓƵ Verification of Illness Form (PDF) with you to be completed by your healthcare practitioner.Ìý
  3. Submit your completed form to the .Ìý
  4. Refer to the  for next steps.Ìý

There is no need for students who have seen a healthcare practitioner off-campus to have their Verification of Illness certificates verified by an on-campus healthcare practitioner.Ìý
