These forms are presented in the most common chronological order of use. Forms not directly available from this page are accessed through the . Go to Content and then Forms. The form you need will be there.

  • Advisory Committee Appointment Form
    • must be approved before a committee meeting is scheduled
    • to be completed within first 6 months of studies
  • MSc Committee Meeting Report Form
    • must occur at least once per year
    • request room and Skype if necessary from Physics Grad Office
    • Form can be found on our LEARN site under FORMS
  • PhD Committee Meeting Report Form
    • must occur at least once per year
    • request room and Skype if necessary from Physics Grad Office
    • Form can be found on our LEARN site under FORMS
  • Graduate Reading Course Request Form
    • to be completed at start of term
  • Nanotechnology Seminar Requirement Form
    • only for students in the Nanotechnology specialization
    • completion required for graduation
  • Quantum Information Seminar Requirement Form
    • only for students in the Quantum Information specialization
    • completion required for graduation
  • Request for Comprehensive Examination Committee
    • must be submitted and approved before the exam takes place
    • review the committee member composition carefully!
    • must log in with WATIAM credentials to complete
    • when selecting examination time, note that your committee will join you 2 hours later!
  • External Examiner Nomination Form (PhD students only)
    • submitted four months before defense date
    • to be submitted by the student's supervisor
    • External examiner must be at arm's length!
  • Thesis Defence Request Form
    • complete to schedule a MSc or PhD defense
    • complete only when dates and times are confirmed with committee members
    • no defenses during Thesis Blackout:
    • must log in with WATIAM credentials to complete
    • to request a room (PHYS 308, 352 or 374) for defense email physicsgradoffice@uwaterloo.ca
    • once defense is confirmed, the date, time or place cannot be changed!!
