Employees asked to check their T4/T4A status

Human Resources has notified employees that they should review their information for 2016 T4 and T4A tax forms, which will be issued later this year.

The tax forms will be produced by February 28, 2017 as required by the Canada Revenue Agency (CRA). In order to ensure that employees receive their forms in a timely fashion, they should review their online consent status, address and Social Insurance Number that Human Resources has on file and make updates if necessary.

Any changes should be made by Tuesday, February 14.

To do this, employees should log into.

Once logged in, navigate to Self Service>Payroll and Compensation>Year End Slips Consent to review consent status.

“If your current status shows “consent received,” then you can view your year-end slips on-line,” says the message from Human Resources. “If your current status shows “no consent received” or “consent reset by employer,” click on the box indicating you give consent and click “submit.” When prompted, enter your password and click “continue.”

To review and/or update their address and SIN information, employees should navigate to Self Service>Personal Information>Personal Information Summary.

Employees should review their address information. If the address listed is not accurate, click on the “Change home/mailing addresses button”; click on the Edit link, update your information and save the change.

Employees should also review their Social Insurance Number (SIN) for accuracy. This is listed at the bottom of the Personal Information Summary screen under Employee Information.

“If your Social Insurance Number is missing (all 9s) or requires a correction, please bring a copy of your card or the SIN slip given to you by Service Canada to the Human Resources Office in EC1 (HR is located on the main floor), or call HR at 519-888-4567 ext. 35935 to provide this information,” says the memo from Human Resources. “We ask that you please do not email your SIN information for privacy reasons.”

Questions regarding updating address and SIN information can be directed topayroll@uwaterloo.caor the Human Resources admin desk at extension 35935.
